This form is for booking the AUDREY II rental package and helps us arrive at a fee schedule for your production. Please fill out the form in its entirety. A Music Theatre International performance license # is required.
Please note: This form does not hold the plants for your requested dates. A non-refundable holding fee is required to lock in immediate booking of rentals. This fee is paid prior to or upon the signing of a rental agreement. The holding fee is deducted from your total rental fees.
All rental pieces for the Audrey II rental package are listed below. Orin's denist chair and gas mask are rented seperately.

ONLY SELF-PICKUP SHIPPING IS AVAILABLE AT THIS TIME. You will need to transport the plants with a U-HAUL truck or similar commercial vehicle from our warehouse location - 17' vehicle recommended. Renter is solely responsible for booking the commercial truck and all financial arrangements. A separate contract is required for self pickup and return.
